ARRM Cares Nomination Form

In order to assist the Selection Committee in the process of selecting several outstanding individuals for the ARRM CARES Award, please describe specific examples of how the nominee demonstrates the following in the box below:

Professional Integrity: Maintaining high standards to benefit consumers, co-workers, and the organization and supporting individuals while maintaining professionally appropriate boundaries;
Networking Supports: advocating to connect consumers with natural support systems;
Creating Innovative, Proactive Approaches to Services: personalizing services, demonstrating an ability to develop and implement new approaches and sharing successful results.

Give specific examples of how this person meets the criteria listed above. Nominations that have won in the past told stories about how the nominee demonstrated exemplary service, and described how the nominee’s skills and characteristics/traits have impacted people’s lives.
